Comprehensive Investigation into the Fungal Hypothesis in Parkinson's Disease: Etiology, Pathogenic Mechanisms, and Methodological Controversies

This narrative review integrates immunohistochemical, genomic, epidemiological, and mechanistic evidence that supports and critiques the hypothesis of a fungal infection of the central nervous system as a contributor to Parkinson's disease (PD). The findings of Pisa, Alonso, and Carrasco on the detection of fungal antigens and DNA in post-mortem brain tissue from PD patients are discussed, along with the proposed routes of entry (hematogenous, intestinal, and olfactory), the pathogenic mechanisms (L-DOPA sequestration by Malassezia, mycotoxins, Sup35 cross-seeding, CARD9/NLRP3 neuroinflammation), and the methodological objections (low biomass, kitome, amplification artifacts) raised by Eisenhofer et al. The synthesis proposes a multi-impact model in which fungal infection acts as a cofactor, alongside genetic susceptibility and gut dysbiosis, rather than as a sole cause.
